Tulsa Curling Club Registration Form
Winter 2012 League Registration League Fees and Information :
Winter League – Ten Weeks (10) – February 25 – April 28, Oilers Ice Center
There is four (4) players per team. All players must be at least 21years of age.
Fees: Ten (10) Week Winter League
Individual – $160
Team – $640
By completing this registration form, I am affirming that I have read, understand and agree to the following:
1. Curling is a sport that involves risk of injury to my person as well as loss or damage to property;
2. I knowingly and freely assume all such risk; and
3. I will sign a Participant Release as provided to me by the Tulsa Curling Club and the Oilers Ice Center.
4. The amount tendered with this application guarantees a spot on a team in the Tulsa Curling Club with the following restrictions:
a) There are a limited number of positions within the club, but early payment will assure placement on a team, based on the order in which payment is received;
b) Payments received after available spots are filled will be returned with no guarantees of league membership; and
c) Once accepted, payments are non-refundable.
5. To play in any league for the 2012 season, I must be a member in good standing of the Tulsa Curling Club.
